Application Checklist
Safe Start: Promising Approaches for Children Exposed to Violence

All applications must be submitted electronically through the Office of Justice Programs’ Grants Management System (GMS) by 8:00 p.m. ET on September 10, 2004:

_____ DUNS Number. Beginning October 1, 2003, a DUNS number must be included in every application for a new award or renewal of an award. Applicants can receive a DUNS number at no cost by calling the dedicated toll-free DUNS number request line at 800–333–0505.

_____ GMS Registration. Registration must be completed by September 8, 2004.

_____ Application for Federal Assistance (SF–424). This form is generated by completing the Overview, Applicant Information, and Project Information screens in GMS.

_____ Assurances and Certifications. The Assurances and Certifications must be reviewed and accepted electronically by the authorizing official or the designated authorizing official.

_____ Budget Detail Worksheet. The Budget must include a worksheet that shows the nonfederal match and a narrative that provides justification for all proposed costs.

_____ Program Narrative. The Program Narrative must include the following items:

_____ Problems To Be Addressed.
_____ Goals and Objectives.
_____ Project Design.
_____ Data Collection and Submission Requirements.
_____ Organizational Capability and Management.

_____ Other Program Attachments. The Other Program Attachments must include the following sections in a single file:

_____ Personnel information.
_____ Memorandum of Understanding.
_____ Supplemental material demonstrating organizational capability/readiness.
_____ List of local leaders and letters of commitment.
_____ Timeline/milestone chart.
_____ Data source and collection information.
_____ Logic model.

Please note that files attached to applications submitted online in GMS must be in the following formats: Microsoft Word (.doc), PDF files (.pdf), or text documents (.txt). Refer to corresponding sections in this program announcement for more detailed information about the required contents of each attachment.
